ADULT COMMUNITY BASED SERVICES

BRIEF DESCRIPTION: CBS is made up of four programs serving the mentally ill in their home communities. The goal of these programs is to assist the individual to live and to maintain themselves in their communities and reduce the need for further hospitalization as much as possible. Programs available are: Targeted Case Management, Community Support Program (CSP), Adult Rehabilitative Mental Health Services (ARMHS). Each program has specific criteria for entrance into the program. Please call for further information on the programs. Crisis Services are being implemented in 2008.

MINNESOTA HELP - 211

BRIEF DESCRIPTION: In 2003, the Minnesota Board on Aging, in partnership with the DHS, launched MinnesotaHelp.info, a web site with thousands of resources to help people find services to meet their needs. The MinnesotaHelp.info database has comprehensive resource information for seniors, caregivers and persons with disabilities. Expansion to include resources for families and children, as well as all Minnesota Medical Assistance and Medicare providers is scheduled to be completed soon. Planning has begun to integrate MinnesotaHelp.info into Phase II of Health Match and also use it in the implementation of the Aging and Disability Resource Center grant activities with Hennepin County. Projections for 2004 indicate that there will be 27,000 unique visitors to MinnesotaHelp.info.
